The establishment observed had oyster tags, but no last sale date on the tags was documented; observed food stored on the floor inside the walk-in freezer; observed mixed seafood inside cold rooms stored uncovered and unprotected; observed raw fish being stored above open containers of caldo de camaron; also observed raw meat stored on top of cooked chicken nuggets and sliced ​​vegetables; the facility observed did not have chlorine or quat sanitizer available throughout the facility; ice maker observed next to walk-in freezer with substantial amount of mold-like substance and debris in dispensing area; observed an employee warming up rice and dropping it into the rice warming unit, the temperature was 115°F; the establishment observed does not respect any dating or labeling; observed chemicals such as Fabuloso stored next to foods such as spices and onions under the prep table; observed several chemical spray bottles with no chemical identification or common name; observed raw shrimp in a three-compartment sink thawing to room temperature; did not observe any ambient air thermometer; observed food containers containing spices and other food products without identifying the common name of the food; observed no evidence of licensed pest control services; substantial discrepancy observed at the rear door; observed inappropriate beverages (no lid, no straw) stored throughout the establishment; observed that no quat or chlorine test strips were available; several drains observed with missing drain covers; observed the hood vent and filters with a significant amount of grease debris; observed personal items stored throughout the restaurant; observed employees switching raw seafood tasks without washing their hands; observed that there was no hand soap or paper towels at the hand washing station.

Several violations of priorities and priority foundations which resulted in a demotion of the installation to unsatisfactory.
